Name the object, state the consequence, focus Cancel
Destructive copy names the object and what happens to it, in the dialog body. Never in the row label: a row that says "Delete" can be triggered by accident from across the room.
Cancel takes focus first. A D-pad has no pointer to aim, so the safe action sits under the cursor.

Delete carries danger on border and label, stepping to dangerFocused on focus.
The full raw filename goes in the body. It wraps, which is correct: the user confirms against the exact file.

Do not put the destructive verb in the row that opens this. The row says what the file is; the dialog says what happens to it.

Do not default focus to Delete: the click you save is the one the user did not mean to make.
